Nurse Practitioner- Primary Care (Rockford Clinic)
The provider works as a member of a multidisciplinary health care team, collaborating with other members of the team to provide medical care to veterans. The provider is expected to demonstrate advanced competencies and skills in assessing the physical and psychosocial status of men and women patients, plan an active role in the multidisciplinary Patient Aligned Care Team, promote preventive health measures, manage the acute and chronic conditions of his/her assigned panel of patients and those patients assigned to other team members when circumstances require. Provide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examinations, and patient education to veterans in the Primary Care Team setting. Functions as an integral part of a Primary Care Patient Aligned Care Team to coordinate, facilitate, and provide patient care and services. Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral. Establish and coordinate a plan of care related to patients identified problems. Validate the effectiveness of previously implemented medical treatment. Record/document health appraisal information, clinical assessments, treatment decisions, plan of care, ongoing management and therapeutic outcomes. Recording will be accomplished using the electronic comprehensive patient record system (CPRS), and will be completed on the day of the encounter. Follow Performance Measure guidelines. Work efficiently with patients, families, members of the multidisciplinary team, and other facility staff. This position is for a float provider and will require some travel to the Freeport, Janesville and Madison facilities to provide coverage for staffing shortages. Work Schedule:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) Current, full, active, and unrestricted registration as a graduate professional nurse AND nurse practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Possess a master's degree from a program accredited by the NLNAC or CCNE and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
