Physician- Primary Care (Rockford Clinic)

The provider works as a member of a multidisciplinary health care team, collaborating with other members of the team to provide medical care to veterans. The physician is expected to demonstrate advanced competencies and skills in assessing the physical and psychosocial status of men and women patients, plan an active role in the multidisciplinary Patient Aligned Care Team, promote preventive health measures, manage the acute and chronic conditions of his/her assigned panel of patients and those patients assigned to other team members when circumstances require. Provide comprehensive assessment, diagnostic work-up and clinic intervention, including comprehensive patient assessment, health history, physical examinations, and patient education to veterans in the Primary Care Team setting. Functions as an integral part of a Primary Care Patient Aligned Care Team to coordinate, facilitate, and provide patient care and services. Order and evaluate diagnostic tests as appropriate, recommend treatment and/or referral. Establish and coordinate a plan of care related to patients identified problems. Validate the effectiveness of previously implemented medical treatment. Record/document health appraisal information, clinical assessments, treatment decisions, plan of care, ongoing management and therapeutic outcomes. Recording will be accomplished using the electronic comprehensive patient record system (CPRS), and will be completed on the day of the encounter. Follow Performance Measure guidelines. Work efficiently with patients, families, members of the multidisciplinary team, and other facility staff. This position is for a float provider and will require some travel to the Freeport, Janesville, and Madison facilities to provide coverage for staffing shortages. Work Schedule:

United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. Proficiency in spoken and written English. Physical requirements outlined below. Reference:

Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
